Aerial Class Liability Waiver

1. Assumption of Risk ｜ 风险承担
I understand that aerial arts involve inherent risks, including but not limited to falling, slipping, bruising, strains, sprains, impact injuries, or other physical harm.

I voluntarily participate and accept full responsibility for my own safety.
我明白空中运动本身具有一定风险，包括跌落、滑倒、瘀伤、扭伤、撞击或其他身体伤害。
我自愿参与，并对自身安全承担全部责任。

 

2. Release of Liability ｜ 免责条款
I agree that the instructors and the studio shall not be held liable for any injury, accident, or loss that may occur during my participation.

I agree not to pursue any legal claims against the instructor(s) or the studio.
我同意任何因上课、练习、拍摄或使用场地而造成的意外、受伤或损失，导师与工作室不承担任何责任。
我不会向导师或工作室追究任何法律责任或索赔。

 

3. Health Declaration ｜ 健康声明
I confirm that I am physically fit to join aerial activities.

I agree to inform the instructor if I have any medical conditions.

If I choose not to disclose, I accept full responsibility for any resulting incident.
我确认我的身体状况适合参与空中运动。我同意如有健康问题，会主动告知导师。若我选择不申报，我将自负其责。

 

4. Safety Rules & Crash Mat Usage ｜ 安全规则与防摔垫使用
The studio provides crash mats for safety.

I agree to use crash mats at all times during practice, filming, or training, especially in the high-ceiling area.

Failure to follow safety instructions is solely my responsibility.
工作室已提供防摔垫。无论练习、拍摄或训练，我同意全程使用防摔垫，尤其是在高天花区。若我不遵守安全指示，责任由我自行承担。

 

5. Equipment & Rigging ｜ 器材与挂点使用
I agree to use only approved equipment.

I will not adjust, move, or alter the rigging system without permission.

I accept full responsibility if misuse results in injury or property damage.
我同意只使用经允许的器材。我不会擅自移动、更改或调整吊点。若因我不当使用导致意外或损坏，我将自行负责。

 

6. Personal Belongings ｜ 个人物品
The studio is not responsible for lost, damaged, or stolen personal items.
工作室不负责保管及赔偿任何遗失、损坏或被盗的个人物品。

 

7. Photography & Recording ｜ 拍摄声明
I understand that classes may be recorded for safety or educational purposes.
我明白课堂中可能会有拍摄用于安全或教学用途。

8. Agreement ｜ 协议生效
By registering, I confirm that I have read, understood, and agree to this waiver.
